I am trying to locate a doctor who is an Anesthesiologist (Acupuncturist & Pain Specialist).  I think he moved out of state and I need some medical information from him.  Please help me if you can.  I would greatly appreciate any suggestions you may have.  I am not a nut, just a patient in need of some information.
