interesting.. how about WPW, or reentry tachycardia OR sinus tachycardia? have you tried ivabradinum?

depending on the site of herniation. transcranial is a universal term, whereas transcalvarian points to the upper half of the cranial fossa :)

i agree with jafo, but might add: why don't you just turn up your sevoflurane, if it's an unexpected, short term prolongation of your attended surgery?

i've thought about that too. small diameter --> flow turbulence --> hypercoagulative state?

personally, we use a manometer attached to the ETT or LMA and mostly 50% N20 in inhalation anaesthesia and the cuff does inflate :)
