Are you serious?

It sure is going to be hard for the pt to breath spontaneously with their chest open.  Remember, the diaphragm works by causing negative intrathoracic pressure which expands the lungs.  Whats going to expand the lungs without the closed chest cavity?

Are you planning on ventilating through the LMA?  If so then just put a tube in.

Both multi trauma and blood transfusions lead to ARDS.

This is an important question in regards to how you handle this. 

Are you a hospital employee or a private contractor?

If you are an employee, you can submit a complaint to the administration.  The complaint will need to mention "disruptive physician".  Your hospital must have a disruptive physician policy.  In this letter you need to mention "hostile work environment".  This ought to get hteir attention.  If it doesn't then you need to decide whether it is worth getting an attorney or letting it ride.  The term "hostile environment" shoulder get the attention of any CEO worth their salt.  The bottom line is to write letters and have situations documented.

Nice Halo,  I use ketamine quite often but never for tourniquet pain.  I can't wait to try it.

I can't think of a reason for a spinal to cause this on one side and not the other.  It is more likely related to the surgery or to a decreased perfusion to the extremety.
