we received a lady 81yr old with #it rt.side. she is a known c/o COPD.
P=110,BP=90/56 mmHg. SpO2 on RA=90%.chest= b/l ronchis+nt.
cvs=diastolic murmur +nt. spine=gross kyphoscoliosis.airway=edentulous.
lab...hb=7.2gm%.Na=127mEq.k=3.6.other parmameters normal.

she is posted for DHS.

how u will go ahead for this case.

is propofol controversy still exists for pregnant pt?is it safe to use in the first trimester of pregnancy?can the standard anesthesia practice for GA induce premature labour?

doing long ortho cases entirely under spinal is sometimes not possible.u have to activate the epidural after some time the block has regressed.then how do u activate the epidural and with what dose?how do maintain the desired level of anesthesia?

recently we performed two craniotomies..both were delayed decompressive craniotomy.they presented late to our hospital.intraop was uneventfull.post op both the pt had severe hypotension. dint responded to fluids.needed Nad and dopamine infusions.stabilized after 24 hr.plz tell me what may be the cause. how to manage such cases..intra/post op.

what will be ur line of m/t for a pregnant lady in first trimester presenting for emergency surgery? what are ur choice of drugs and their effect on the developing fetus?

when using cse for any long surgery(lower limb/bladder reconstruction etc) when do u activate the epidural and how do u dose it?what decides the epidural top ups?
