hypoxemia

hey
i am a veterinary surgeon. during an open chest procedure in a 85kg sheep, a slow process of hypoxia occurred. the animal was ventilated, the lungs were seen with a nice pink color and with nice feeling. the saturation was decreased, new intubation was made, new oxygen supply source was used with bag (instead the ventilation), yet the oxygen was reduced(30%) and the co2 was elevated (60%). the sheep died eventually. it should be emphasis that a millar catheter was inserted to the left atrium.
any suggestions what have happened???
