Upper BMI Limit for GA in a free-standing Ambulatory Surgery Setting

Just wanted some input from other Anesthesiologist about their BMI criteria at the surgical centers they provide care at with regard to BMI.
Currently, at our single OR ASCs, where we mostly do Knee Scopes and GU cases, we have set an upper BMI limit for GA cases at 40 and will go up to BMI 45 for MAC sedation cases.  Our surgeons want us to increase the BMI limit for GA to 45 with the added screening that all patients with BMI 40-45 have to come in for an airway exam and those with diagnosed sleep apnea are automatically not candidates for GA at the ASC.
So what do you guys think should be the BMI cutoff for GA at a surgical center, if any?  What criteria are you using at your ASCs?  Please let me know as I have checked the lierature and have found very little consensus on this issue.
